Molar intrusion and comprehensive orthodontics

Example appliance: TAD molar intrusion appliance (Benedict mouse-trap appliance) with fixed appliances

Level of evidence: Moderate
Suitable for ages: 12+ years old
Indications: Anterior open bite, mild to moderate over-eruption of posterior teeth, ideal or increased incisal display, vertical maxillary excess, dental Class II occlusion, down and backward rotated mandible, skeletal open bite/hyperdivergent, convex facial profile
Contraindications: Hypodivergent skeletal type, deep bites, acquired orthodontic or TAD-based risks

Treatment Objectives and Biomechanics

  • Intrude molars and posterior teeth
  • Autorotation of the mandible forward and upwards
  • Reduce anterior open bite
  • Maintain upper incisors vertical position

Appliances / Treatment modalities

  • Skeletal anchorage – TADS, bone plates
  • Extra-oral anchorage – High pull headgear (weak evidence of limited true molar intrusion capabilities)
  • Clear aligners – (weak evidence of limited true molar intrusion capabilities)
